Upon provision of services, the medical institution must request medical form No. 100 /A and the original document confirming payment for services (cash receipt).
Exclusions from insurance coverage
Pre-existing diseases
Insurance does not cover chronic diseases and their exacerbations, with the exception of urgent, life-threatening cases.
Deliberate self-harm
Cases involving intentional harm to health are not covered (except for situations of saving someone else's life).
Accidents related to alcohol and drugs
Injuries sustained under the influence of alcohol or drugs are not eligible for compensation.
Professional and extreme sports

Aesthetic medicine and dentistry
The policy does not cover cosmetic procedures, including plastic surgery, vision correction and infertility treatment.
